New Patient Checklist

It is crucial when choosing a pediatric practice for your children that you have given serious consideration to the fit between the practice philosophies and your own.

Excellent care happens when the physicians and office staff are aligned with your family priorities and philosophies regarding medical care. Only then can shared decision-making be based on mutual respect.

Please read the following checklist carefully to be sure we are the appropriate medical home for your children.

First Appointment

A few weeks before delivery, call the office to let us know you are expecting. You may want to download and fill out our paperwork at that time. When the happy day has arrived, you can tell the hospital staff that you have chosen us as your pediatrician.

What to Bring

• Any paperwork from the hospital, including hearing screen results and immunization records.
• Any available insurance information (often only the parent?s insurance is available at the first visit).
• Plenty of diapers and an extra set of clothes.
• Please complete the new patient forms (add link to welcome packet PDF) prior to the visit to help make the appointment as smooth as possible.
