It is a pleasure having you choose this medical practice for your health care needs. This information pamphlet is provided
to introduce you to the practice in addition to answering many frequently asked questions.
Please read this brochure in it's entirety and retain it for future reference.

We thank you for entrusting the health care of your children to our practice. Our goal here is to provide quality traditional
medical care to children from birth through age 18. In the pursuit of providing each child with comprehensive medical care,
this office offers well child check-ups, sick visits, sports physicals, routine immunizations and limited procedures. Our
practice here also has the goal of 99% of its clients vaccinated against common childhood infectious diseases. We believe
this to be one of the most important services health care givers provide.
Office Hours and Appointments:
Patients are seen by appointment only, so please out of respect for the other scheduled patients, call in advance prior
to bringing your child in. In addition, please advise our front desk if another child needs to be evaluated.
If you are a new patient or have not been seen in this office in over three (3) years, please plan to arrive twenty (20)
minutes prior to your appointment. Please bring your insurance card, drivers license and your child's immunization records.
If you are unable to keep your scheduled appointment time, we ask that you call ahead at least 24 hours in advance. This
allows the office to maximize appointment availability for other patients.
If you are more than 30 minutes late for your appointment time, you may be asked to
re-schedule. If you have not shown up for and/or are more than 30 minutes late on at least three (3) occasions, your ability
to pre-schedule appointments may be limited.
If you have not been called back to an exam room after 20 minutes, please let the receptionist
know. Every effort will be made to keep you informed should an emergency occur and prolong your wait.
Office Visit Authorization For Treatment:
When possible, we request that a parent or legal guardian accompany your child to our office. We do require a parent or
legal guardian be present at the first office visit since a complete medical history must be obtained.
Our office will provide you with an authorization form for you to specify those whom you empower to make
medical decisions on your behalf in your absence (i.e. grandparents, step-parents etc ... )
If your child requires a referral authorization by your insurance company prior to seeing a specialist,
this office requires at least three (3) business days notice prior to the appointment with that specialist.
Medication Refill Requests / School Forms:
When you call during business hours for medication refills, please leave your name, your childs
name, date of birth, medication name, dosage and frequency of requested medications. We will also need the name and telephone
number of your pharmacy. Please allow the office 24 hours for prescription refills. All prescriptions for controlled
medications can only be picked-up during office hours.
Please call for your refills prior to your last dose of medication, to avoid any interruption of your treatment.
If your child requires a health physical form or immunization record (blue and gold forms) in order to attend school/day
care, please allow the office 48-hour notice for these forms to be completed. You may also request these forms or other forms
when your child sees the physician at his/her checkup, without charge. However, there may be a charge for repeat requests
to complete such forms other than during an office visit.
After Hours Emergencies:
Our office provides 24 hour on-call service for urgent issues only. All non-urgent problems,
routine advice, routine refills, etc., will only be handled during office hours. If you call the office for urgent problems
after hours, you will be connected to our answering service. If you do not receive a call within one (1) hour, please do call
